REVIEW WHAT STILL COMES BACK Review period: Question/case reference: Note version used: What happened: Was the barrier information, guidance, authority, practice or a real exception? Was the customer updated as agreed? What needs changing: Change owner and due date: Review decisions and follow-through, not only the number of escalations. A justified escalation is useful. Do not reward silence or unsupported guesses. Do not treat a transcript or AI summary as approval for a business rule.
